The medical team carefully balances hormone levels to optimize the number and quality of eggs while minimizing the risk of ovarian hyperstimulation syndrome (OHSS).

Egg Retrieval

When ultrasound monitoring and blood tests confirm that the follicles have reached optimal maturity, an HCG “trigger shot” is administered to stimulate ovulation and initiate the final stage of maturation. Approximately 34–36 hours after the trigger shot, the patient undergoes a precise, minimally invasive surgical procedure known as transvaginal ultrasound-guided oocyte aspiration (also called egg retrieval). Performed under sedation, this 20–30-minute procedure involves guiding a thin needle through the vaginal wall into each ovarian follicle. The physician aspirates the eggs from the follicles using a suction device, while an embryologist examines each egg to assess its maturity and quality. Most patients can return home within a few hours of the procedure, though they may experience mild cramping and discomfort for 24–48 hours.

Fertilization

In the controlled environment of the embryology laboratory, each mature egg is carefully prepared for fertilization. The most common fertilization method is intracytoplasmic sperm injection (ICSI), in which a single sperm is directly injected into each egg. Traditional insemination may be used in cases with normal sperm parameters. The embryologist maintains precise temperature, humidity, and pH levels to create optimal conditions for fertilization. Within 16–18 hours, the eggs are examined for signs of successful fertilization, marked by the presence of two pronuclei.

Embryo Development

Following fertilization, the embryos enter a critical development phase that typically spans 5–6 days. During this period, embryologists monitor their progress through various developmental stages, from initial cell division to blastocyst formation. Advanced time-lapse imaging systems track each embryo's development, allowing embryologists to assess factors such as cell division timing, symmetry, and fragmentation. This detailed monitoring helps identify the embryos with the highest potential for successful implantation. By day 5, 6, or 7, viable embryos should reach the blastocyst stage, containing approximately 200–300 cells.

Embryo Transfer

The embryo transfer represents the culmination of the IVF process, requiring precise timing and technique. Using ultrasound guidance, the physician carefully places one selected embryo into the uterus through a thin, flexible catheter. This procedure, while typically brief and painless, demands extreme precision to optimize implantation chances. The number of embryos transferred is determined through careful consultation between the medical team and patient, considering factors such as embryo quality, patient age, and previous pregnancy history. Any additional viable embryos will be cryopreserved for future use.

Pregnancy Test

The two-week period following embryo transfer, often referred to as the "two-week wait," concludes with a quantitative beta-hCG blood test to detect pregnancy. This highly sensitive test measures the exact levels of human chorionic gonadotropin (hCG), providing clear evidence of successful implantation. Patients typically undergo a series of these tests over several days to confirm appropriate hormone level increases. Once hormone levels are confirmed, an OB ultrasound is usually performed at approximately 6 weeks of pregnancy (or 3–5 days after the missed period) to confirm a viable pregnancy. If successful, the clinic monitors the pregnancy until approximately 8–10 weeks gestation, at which point care transitions to a regular obstetrician.

Understanding the Risks and Benefits of IVF

As with any medical procedure, IVF carries certain risks and potential side effects. These may include ovarian hyperstimulation syndrome (OHSS), multiple pregnancies, and emotional stress.
